Description (Service)	Provides nursing care and personal support care PSW level or Nursing level geriatric trained staff are certified to provide care to clients with Acquired Brain Injury and to clients with Dementia Nursing Care Services include: palliative care pediatric care wound care mental health care post-operative care medication management health teaching including wound and stoma therapy advice and Nurse Continence Advisor services physical, mental health and mobility assessments medication and nutrition review Personal Support Care services include: light housekeeping meal preparation personal care friendly visiting companionship and accompaniment to appointments respite care FALLS PREVENTION: Fall prevention assessments in the client's home Member of Simcoe Muskoka Integrated Fall Strategy (SMIFS)
