    Hours Mon-Fri 8:30 am-4 pm, hours may vary
    Areas Served Dufferin County ; Hamilton (City of) ; Niagara Region ; Northumberland County ; Peterborough (County of) ; Simcoe County ; Wellington County
    Eligibility Legal spouse or common-law partner of the deceased CPP contributor.

      • paper application forms can also be picked up at Service Canada Centres or Funeral Homes
      • don't delay in sending completed application forms, delay may cause a loss of benefits
