Description (Service)	Home health care services including: Nursing Care Caregiver Relief Pediatric Care Elder Care Palliative Care ET Nurse Continence Advisor Post-Operative Care Chronic Disease Management Education Personal Support Services such as Companionship, Dementia Care, Physiotherapy and Exercise Programs Falls risk assessments, work with clients to further reduce the risk of a fall by providing: Nurses and PSW workers to manage and educate clients on individual medical risk factors such as Diabetes, advanced foot care and bladder and bowel incontinence Visiting nursing to monitor medications use Product advice and help with product use, including assessments for assistive devices
