Description

Description (Service)

StreetReach is a mobile, low-barrier, assertive street outreach program serving individuals experiencing homelessness, unsheltered living, mental health concerns, or housing instability. The team provides seven-day-a-week support, including housing-focused case management, system navigation, and referrals to shelter, housing programs, income supports, health services, and community resources.

Services include:

  • Housing-focused case management

  • Navigation to shelter, housing, income supports, and community services

  • Basic needs (food, water, hygiene supplies)

  • Blankets, clothing, and outdoor/survival gear

  • Bagged meals (lunch and dinner)

  • Tents, tarps, and weather-appropriate supplies

  • Access to showers (Barrie - 88 Mulcaster St)

  • Engagement, conversation, and wellness checks

  • Minor first aid

  • Harm-reduction supplies

  • Mental-health-informed support

  • Brief crisis support and de-escalation

  • Advocacy and referrals
