Description (Service)	Patient-centred education and support to individuals living with Type 1 Diabetes, Type 2 Diabetes, Prediabetes or Gestational Diabetes Consists of a team of Endocrinologists, Registered Dietitians (Certified Diabetes Educators) and Registered Nurses (Certified Diabetes Educators) Provide information and education around healthy eating, exercise, monitoring blood sugar levels, role of medications and insulin we well as other lifestyle factors Insulin pump program site
Hours	Mon-Thu 8:30 am-4:30 pm, Fri 8:30 am-3 pm

Eligibility	Adults 18 years and older with Type 1, Type 2 or Gestational Diabetes as well as those with Prediabetes
