Description

Description (Service) Helps make the transition from hospital to home
  • Provides transportation and the services of a Home at Last (HAL) Attendant (PSW)
  • Once at home the HAL Attendant can provide assistance with groceries, medication and medical supplies pick up, light housekeeping, laundry and meal preparation
  • HAL Attendant will provide the client with a package of community support services
  • Followup within 24-48 hours by HAL supervisor
