  • must have referral from Barrie Family Health Team doctor or Nurse Practitioner and need to access two or more of their services
  • help patients reduce risk and navigate to overcome barriers
  • health care team includes Registered nurse/Patient Navigator, Pharmacist, and an Occupational Therapist
