Description (Service) |
Provide information, education and support for persons living with dementia and their care partners. The following programs and services are offered:
- Navigation (connects individuals newly diagnosed and/or who have not accessed other support / community services)
- Supportive Counselling
- Care Partner Support Groups
- Education programs for persons living with dementia, their care givers, professionals and the general public
- Social, Recreational and Exercise programs
- Resource Library & Information packages
- Activity Kits

Application |
- Call, email or drop-in to access general information and/or to start the process of accessing the services
- Appointments are required to speak with a Family Support Coordinator
- Members of the public can self-refer and family members can refer loved ones by contacting the office
- Community partners and Family Physicians can refer clients by using the First Link Referral form
