| Description (Service) | Health centre providing family medical care and community-based health programs * team of nurses and other health care providers * referral to other regional health services Culturally appropriate programs and services for Aboriginals, combining traditional health and Western medical practices * aims to improve the social supports and conditions that affect long-term health Provides a variety of health services including: * Home and Community Care * health information * oral health * counselling services * nutrition services * Aging at Home support program * Healthy Babies/Healthy children program * Christian Island addiction and mental health program Medical transportation program - Coordinates medical transportation services and assists with medical appointments in other communities Circle of Care - Programs and services to help individuals remain independent as long as possible at home in their own community * services include personal support work, nursing, social work, respite care, palliative care, homemaking, educational and wellness programs and functions, day services (ages 55 and over), meals on wheels Tuesdays and Thursdays (only those eligible based on medical and financial assessments), maintenance (minor repairs), loan cupboard – medical assistive devices, recreational activities and referrals to other service providers |
