Description (Service)	Medical care team of health professionals including Nurse Practitioners, Social Workers, Nurse Educators and Dietitians providing comprehensive care using an interdisciplinary approach. Programs and services: cardiovascular rehab diabetes dietitian heart health health promotion (hypertension, injection clinic, smoking cessation and walking for health) lung health education (asthma, COPD and pulmonary rehab) mental health (adult mental health, after baby group, children's mental health, get fit while you sit, increasing access to structure psychotherapy, mood management group, single session) senior care program senior CARE exercise telemedicine well baby for infants from birth to two years well child for children two years of age and prior to starting kindergarten In partnership with: Chigamik Community Health Centre (CHC) Georgian Bay General Hospital (GBGH) North Simcoe Ontario Health Team (NSOHT) Wendat Community Programs
