Telehomecare supports patients living with conditions such as chronic obstructive pulmonary disease (COPD) or congestive heart failure (CHF) through health coaching and remote monitoring

• patients are provided with easy-to-use home-based equipment, allowing them to monitor their own vital signs and answer simple questions about how they are feeling

  • District of Muskoka
  • Simcoe County - all areas north of Hwy 89
  • Grey County - small portion of eastern Grey County, including Maxwell and Feversham
• Patients that have been diagnosed with COPD and Heart Failure that have either a landline or cell phone maybe eligible for this service
• Age Requirements:18 year(s) and up
