Description & Services
Description (Service) | Outpatient Assessment and Treatment Service (OATS) provides a goal-directed service for individuals experiencing moderate to severe mental health challenges where care needs cannot be met through other community resources. Services through OATS are time limited (6-9 months) and may include psychiatric consultation, as well as group therapy and individualized supports for treatment and recovery. Services may include:

Eligibility | Individuals 16 years and older experiencing moderate to severe mental health conditions. |
Application | Referrals to OATS must be completed by a Primary Care Provider using the Waypoint Referral Form. Completed referral forms need to be submitted to Waypoint's Central Intake department. Following acceptance to the OATS program, a Clinician will contact the client directly to schedule an Intake Assessment. |
