Description & Services

Description (Service)	Outpatient Assessment and Treatment Service (OATS) provides a goal-directed service for individuals experiencing moderate to severe mental health challenges where care needs cannot be met through other community resources. Services through OATS are time limited (6-9 months) and may include psychiatric consultation, as well as group therapy and individualized supports for treatment and recovery. Services may include: Psychiatric consultation, assessment, and treatment, including diagnostic clarification and medication stabilization Medication therapy and monitoring Evidence-informed psychotherapeutic and psychoeducational groups Interprofessional support Advocacy and connection to other community resources Discharge planning
Hours	Mon-Fri 8 am-4 pm
Areas Served	Dufferin County ; Muskoka District ; Simcoe County
Eligibility	Individuals 16 years and older experiencing moderate to severe mental health conditions.
Application	Referrals to OATS must be completed by a Primary Care Provider using the Waypoint Referral Form Completed referral forms need to be submitted to Waypoint's Central Intake department. For more information regarding referrals, visit the website or contact Central Intake at 705-549-3181 ext 2308. Following acceptance to the OATS program, a Clinician will contact the client directly to schedule an Intake Assessment.
